  1. Abstract of 300 words or less outlining the rationale for the proposal, methods, and how results of this pilot research will assist you in obtaining peer-reviewed external funding.
  2. 5 page NIH biosketch for you and each of your faculty collaborators
  3. Budget with justification.
  4. 2- 3 page proposal that outlines:
    • Background including issues you are addressing
    • Goals and Specific Aims of this proposal
    • Significance: How this research will improve the field and /or provide data to support further research.
    • Approach and Methods including plans for analysis
    • Description of Investigator(s) including a summary of relevant experience in this area
    • Plans for Future Funding including specific grants, agencies, and projected timing of application submission.
  5. Pages that may be added after the proposal, at the discretion of the investigators:
    • Up to one page of references can be placed at the end of the proposal
    • Up to one page of figures with legends and/or tables may be included after the references


Any publications supported by this program must cite “Charles E. Putman MD Vision Award, Department of Radiology, Duke University School of Medicine” in the acknowledgements.
